Discharge Planning

Discharge planning begins at admission. The case manager or social worker you meet will assist you and your family with all discharge needs throughout your stay with us, including the ordering of equipment and providing referrals to home care, outpatient therapy or community services.

Patients and their family members are an important part of the rehabilitation team. By participating in your rehabilitation program, your family will be better prepared to help you once you return home. Your therapy sessions will be intensive and time consuming, and we will ensure to coordinate training time with your family so they can learn how to safely assist you in your home environment.
