DeGraff Receives 2016 SOPHi Award

The 2nd annual Spotlight on Population Health (SOPHi) awards were held on Friday, October 14, 2016 at Buffalo RiverWorks along the banks of the Buffalo River.

SOPHi is a celebratory event, showcasing the great work that is being accomplished in Western New York. DeGraff Memorial Hospital received the award in the prevent injury category for the Stay Well On Your Feet program offered throughout Niagara County. 

The focus of the Stay Well program is to educate older adults on self-management of behaviors geared toward good health and wellness. Stay Well On Your Feet sessions combine health education presentations, blood pressure monitoring, risk for fall assessments, interactive demonstrations and community resource referrals.

 DeGraff is part of the Niagara County Coalition for Falls Prevention along with the Niagara County Department of Health, Niagara County Office for the Aging and Visiting Nursing Association of WNY.
