Below are some of the most commonly requested employee and associate resources all in one easy-to-access location. Click on each button for more information.

Employee COVID-19 Testing

  • Employees must bring their ID badge at the time of sample collection.
  • Those tested will receive their results through the Kaleida Health Patient Portal.
  • Kaleida Health employees and employees of GPPC, UBMD, and GLMI must make an appointment.
  • Testing sites will be closed Labor Day, Thanksgiving Day, Christmas Day, New Year's Day (open Christmas Eve and New Year's Eve)

MyIT Service Portal

IT Service Portal (MyIT) is available for all Kaleida Health employees to see computer system outages and other notifications, electronically submit tickets, view open tickets, request specific products/services and search for answers.

KPass

KPass is a self-service portal to register employee profiles or create a password as a new user or reset a forgotten password.

Associate Identity Request Form

The Associate Identity Request Form (A.I.R. Form) is used to validate information about a user from a Kaleida Health Partner. When the A.I.R. form is received, CyberSecurity will submit the request for approval to all required entities. Upon approval, the user will receive a Kaleida Health "Associate" number. The "Associate" number will entitle the user to certain resources within Kaleida Health.

Associate Identity Request (A.I.R.) Demographic End User Information

The spreadsheet is used to load multiple users into the AIR system which would facilitate access to Kaleida Health resources. Complete the Demographic Information Tab by downloading the form below and email the completed spreadsheet to your Kaleida Health contact.The spreadsheet and submission must be completed no less than two weeks prior to when access is required.

Kaleida Health Credit Union

Employees of Kaleida Health, Brylin Hospital along with, independent contractors, or self-employed persons who work regularly in the Health Care Industry in the New York counties of Erie or Niagara, and their family members, are eligible to join KHFCU.
