Subacute rehabilitation acts as a transition from hospital to home. Its goal is to build strength, increase mobility to the fullest extent possible, and ensure that the patient can function effectively upon returning home.
Subacute rehabilitation is used for patients who have had orthopedic surgery or injury, stroke, diabetes and other disease states.
