Dear Bariatric Candidate:
Thank you for your interest in the Center for Minimally Invasive Surgery. We appreciate the opportunity to help you reach your weight loss goals.
If you are interested in exploring weight loss surgery, your first step is to complete a Center for Minimally Invasive Surgery (CMIS) application. The application will be sent to your health insurance provider for payment authorization, so it must be filled out completely; with no empty spaces.
Please note that pages seven and eight of the application must be completed by your primary care physician. He/she can either send it directly to us or you can include it when you return the rest of the application. You are welcome to drop off your application to us or send it by fax or U.S. postal service. Once we receive the completed paperwork, you and your support person will be invited to attend one of our mandatory informational seminars. Please refer to the “seminar” link.
After attending the seminar, you will be scheduled for your first clinic visit where you will meet with a nurse practitioner, physical therapist, and dietitian to discuss your weight loss goals, diet plan and work up requirements for surgery. This appointment usually lasts two-three hours.
As a result of this visit, our clinicians will order a personalized series of tests, lab work and evaluations that you must complete. Once a clinician feels you are appropriate and ready for surgery, you will meet your preferred surgeon to discuss surgical options.
