PGY2 Critical Care Pharmacy Practice - How to Apply


  • Graduate of an ACPE accredited School of Pharmacy
  • Successful completion of PGY1 Residency
  • Eligible for NYS Licensure
  • On-site interview with formal presentation

Application Process

An application form is not required, but the following items must be submitted:

  • Letter of intent
  • Three professional references
  • Transcript of university grades
  • Curriculum vitae

Application materials must be received by January 21 of the year you wish to begin the program to be considered for an onsite interview. A formal presentation is required during the interview.
