Applicant must be a graduate of an ACPE accredited School of Pharmacy and must be eligible for New York State Licensure.

Application Process

An application form is not required, but the following items must be submitted through PHORCAS:

  • Letter of intent
  • Three professional references
  • Transcript of university grades
  • Curriculum vitae

Application materials must be received by January 5 of the year you wish to begin the program to be considered for an onsite interview. A formal presentation is required during the interview.
