How to request photocopies of your medical records

There will a charge for copies of records for your personal use. Records will be mailed within 5 business days of receipt of your request.

How to obtain copies of birth certificates
Even though the hospitals of Kaleida collect information, complete the original birth certificates, and submit the information to the New York State Department of Health, the birth certificates are maintained by the Department of Vital Statistics in your city or town. All requests for copies of birth certificates must be made to these departments. There may be a small charge for additional copies of birth certificates.
