Pharmacy Department

The Pharmacy department at DeGraff Memorial Hospital has the major responsibility for procurement, storage and distribution of medications for all inpatients and Skilled Nursing Facility residents. The department provides medication order review and evaluation as well as clinical monitoring, consultative and reporting services. The department also provides medication information to hospital staff and serves as a site for multiple pharmacy educational programs in the area.

The traditional pharmacy function of medication procurement at the DeGraff Memorial Hospital has been enhanced with the addition of online pharmaceutical ordering at the supplier level. The distribution system features automated supply cabinets on each unit as well as unit-dose robotic cart fill based from Millard Fillmore Suburban Hospital. Order monitoring and evaluation are undergoing continuing improvement through efforts with the Kaleida Pharmacy Computer Team.

The DeGraff Memorial Hospital Pharmacy has also implemented a collaborative clinical pharmacy program under the leadership of the Medical Staff and the Pharmacy Clinical Coordinator. The department currently provides pharmacokinetic and clinical antimicrobial consultative services in addition to other specialized therapy consultation for a wide range of medications. The Clinical Coordinator also serves as consulting pharmacist to the DeGraff Skilled Nursing Facility.

Finally, the department serves as a precepting facility in the Kaleida Pharmacy Residency Program and the Doctor of Pharmacy program at the University at Buffalo.
