Health Information Management Department (Medical Records)

The Health Information Management Department provides services to patients, healthcare providers and other authorized requestors, resulting from the management of healthcare data and records generated by patient care services at the Women & Children's Hospital of Buffalo.

Services provided by the Health Information Management Department at WCHOB include provision of an accurate patient record, as supported by:  

  • Record processing (assembly and analysis)
  • Coding, DRG assignment and abstracting
  • Registry and other database management services (Birth Registry, Cancer Registry, Tumor Registry, SPARCS data submission, Hospital-based statistical indices)
  • Centralized physician transcription services
  • Correspondence and release of information
  • Record maintenance (storage and retrieval)

The Health Information Management Department is committed to providing accurate, timely and useful health information to meet the needs of its users. The department supports the mission of Kaleida Health through the capture and storage of key clinical data, the conversion of the data into strategic information and the timely communication of that information to authorized requestors.

To request a copy of your medical record, you may download a Release of Information form from the Kaleida Health website, www.kaleidahealth.org, under the “Contact” tab and mail the completed form to:

How to obtain copies of birth certificates

Even though the hospitals of Kaleida Health collect information, complete the original birth certificates, and submit the information to the New York State Department of Health, the birth certificates are maintained by the Department of Vital Statistics in the person's city or town.

All requests for copies of birth certificates must be made to these departments. There may be a small charge for additional copies of birth certificates.
