Looking for patient information?

Where do I begin?

  1. If you are interested in utilizing Kaleida Health services, please call toll-free at 1-866-878-7006. You will be connected to a customer care representative to assist with your healthcare inquiry.
  2. We will connect you with a physician who can help you with your healthcare needs.
  3. If you are a candidate for a surgical procedure and wish to proceed, we will help to schedule your consult with our specialists and do all we can to make your experience as easy as possible.
  4. If you wish to seek approval from the Ministry of Health, your primary care physician will have to complete and submit a "Prior Approval Application for Full Payment of Insured Out-of-Country Health Services" form.
  5. If the Ministry of Health does not approve your procedure, you will have the option of paying for the procedure yourself.

Why should I pay for care when/if the Ministry of Health will pay?

There are a variety of procedures that either cannot be found in Ontario or the wait time is simply too long. Kaleida Health's five hospitals provide an alternative so you can make your healthcare decision on your terms. It is not unlike the decision many of us make about elective cosmetic surgery, but in many cases there are more immediate improvements to your quality of life. So don't wait if you don't have to.

If the Ministry of Health does not cover your procedure, all self-pay services must be prepaid in full prior to procedure date.

We have created self-pay package prices for a variety of procedures offered at Kaleida Health hospitals, which allows us to include all hospital charges, surgeon charges and anesthesia charges.* For more information contact us at 1-866-878-7006.

*Added charges due to complications are the responsibility of the patient.
