Admissions

The Admissions Department is the entry point for patients receiving any test or treatment at the hospital, including: surgery, labor and delivery, medical treatment and outpatient testing. The admissions staff registers patients by gathering all pertinent demographic and financial information needed to provide services.

There are two ways to be admitted as an inpatient to Millard Fillmore Suburban Hospital.

The first is a physician referral whereby a physician will contact the hospital and provide an order for a person to be admitted.  At the time of admission, the individual will need to provide insurance information and the name and phone number of his or her primary care physician. The Admissions Department will also need to know if the patient has special requirements such as an interpreter.

The second is by going through the Emergency Department whereby after an examination, the ED physician directly admits the person to the hospital.

Preparing for a Visit

Coming to the hospital can be stressful. The following information will help prepare for the visit or stay:

Outpatients

Individuals coming to the hospital for outpatient procedures or testing should bring insurance information, physician’s prescription, if applicable, and a list of all current medications.

Inpatients

Once a person is admitted, a Patient Management Coordinator is assigned to coordinate care from admission to discharge. Patient Management Coordinators help ensure that the special needs of patients and families are identified and addressed appropriately. The coordinators serve as the primary liaison between physician, patient, family and other members of the healthcare team.

Health Care Proxy and/or Living Will

A Health Care Proxy or Living Will (Advanced Directives) is for patients 18 years or older who want to choose another person to act as a representative if they become unable to make decisions about their care. This person would be responsible for making all medical decisions, including those involving life-sustaining medical treatment, in agreement with his or her understanding of the patient’s wishes. If you have prepared an Advanced Directive, please tell a member of your healthcare team when you are admitted. Click here to complete one.

Parking

Parking at Millard Fillmore Suburban Hospital is free. Valet service is also available for a minimal fee.

Pre-Admission Testing
